Please check with your insurance or call our offices to find out if we are part of your insurance network.  It is your responsibility to contact your insurance carrier to determine your benefits and requirements on referrals.  If we do not participate with your insurance carrier, you may want to ask your carrier if you have out of network benefits.  We will be happy to file your claim if you have out of network benefits.  Any deductible or portion due from the patient will be due at the time of service.

Please bring your insurance card and photo ID to each visit.  Copayments are collected at check out; self-pay patients are required to pay at the time of service.
